Clinical Overlap of Vitiligo and Psoriasis: A Histopathologically Confirmed Case Report

Vitiligo and psoriasis are highly prevalent chronic inflammatory dermatoses, usually described independently. Their concomitant presentation in the same patient represents a rare and clinically challenging entity. We report the case of a 50-year-old female patient with achromic macules in atypical locations (dorsum of both feet and the malleolar region), on which well-defined erythematous plaques with thick, adherent scales were observed. Both pathologies share immunological mechanisms mediated by T lymphocytes (Th1, Th17 in psoriasis; CD8+ in vitíligo) and proinflammatory cytokines (IL-17, IL-23, TNF-α e IFN-γ, CXCL10), which contribute to keratinocyte dysfunction and melanocytic destruction, respectively. The treatment was aimed at modulating the immune response using high-potency topical corticosteroids and calcineurin inhibitors (tacrolimus), with favourable evolution and partial repigmentation of the lesions.
