Introduction: Tooth avulsion is one of the most severe forms of dental trauma, characterised by the complete displacement of a tooth from its socket. Successful management depends on prompt intervention, appropriate handling of the avulsed tooth, extraoral dry time, storage medium, and timely replantation. Adherence to evidence-based treatment protocols can significantly improve the prognosis and preserve both function and aesthetics. Case Presentation: A healthy patient presented with an avulsed permanent maxillary left central and lateral incisors following traumatic injury. The avulsed teeth were retrieved, clinically assessed, and managed according to established trauma guidelines. After socket debridement and evaluation of the periodontal tissues, both teeth were replanted under local anaesthesia and stabilised using a flexible splint. Endodontic therapy was initiated at the appropriate time, followed by intracanal medicament placement and definitive obturation. The patient was prescribed systemic antibiotics, advised on oral hygiene measures, and enrolled in a scheduled follow-up protocol to monitor periodontal and pulpal healing. Results: Clinical and radiographic evaluations during follow-up demonstrated satisfactory tooth stability, favourable periodontal healing, and functional restoration. No signs of inflammatory root resorption, ankylosis, or periapical pathology were observed during the follow-up period. The patient regained satisfactory aesthetics and function with successful preservation of the natural tooth. Conclusion: Timely diagnosis, appropriate emergency management, and multidisciplinary follow-up are essential for the successful management of avulsed permanent teeth. Even in traumatic injuries with a guarded prognosis, adherence to current clinical guidelines can facilitate favourable outcomes and maintain the natural dentition. Long-term follow-up remains crucial for the early detection and management of potential complications.
