Nephrotoxicity induced by hyperbilirubinemia is a rare but severe phenomenon, often underestimated in clinical practice. We report the case of a patient with pancreatic cancer whose progression was marked by severe cholestasis and major hyperbilirubinemia, complicated by acute renal failure. This presentation raises the issue of direct bile pigment deficiency in the renal parenchyma, in the context of neoplastic hepatobiliary involvement. We discuss the probable pathophysiological mechanisms, diagnostic features, and available therapeutic options, while proposing avenues for improvement in the multidisciplinary management of this type of patient.
